Cardholder's House Number/House Name
The information you enter here must be the same as that given by the cardholder to the card company, i.e. as it appears on the cardholder's statement. Enter only the number of the house as written in the street address. If the house does not have a number then enter the name of the house.

Example:
Suppose your address is 44 High Street; then you would enter 44 in the box.

Cardholder's Post/Zipcode
The information you enter here must be the same as that given by the cardholder to the card company. Enter the full post code or zipcode as it appears on the cardholder's statement, e.g. CB2 5LR

If you are from an area without a postcode, please enter "none" in this field.

What is the Card Security Code?
For most cards this number appears on the back of the card at the top of the signature strip (we require the last 3 digits only).

On some other cards, including American Express there is a four-digit number on the front of the card just above the account number.

Information regarding the card's date of issue
Many cards these days have a date of issue, similar to that of the expiry date, consisting of a month and a year (eg "04" and "02" for April 2002). This is generally found on the front of the card, beneath the card number, near the expiry date.

On some cards, this information is not present; if this is true for your card, please ignore the field.
